The Assistive Technology Device Predisposition Assessment (ATD PA) is an example. It consists of 66 items divided into four sections:

  • Section A (9 items) asks for consumer subjective ratings of functional capabilities (5-point Likert scale).
  • Section B (12 items) address subjective well-being/quality of life.
  • Section C consists of 33 items that profile an individual's personal and psychosocial characteristics (yes/no).
  • Section D (12 items) asks respondents to rate their expectations of a particular AT (baseline version) or realizations of benefit from an AT (follow-up version).

The specific forms included in the MPT are:

  • Initial Worksheet for the Matching Person and Technology (MPT) Model is used to determine initial goals that the consumer and professional have established. Potential interventions supportive of these goals, and technologies needed to support the attainment of the goals, are recorded.
  • History of Support Use is used to identify supports used in the past, satisfaction with those supports, and those that are desired and needed but not yet available to the particular consumer.
  • Specific technology matching: The consumer is asked to complete his or her version of the appropriate form depending on the type of technology under consideration:
    • General: Survey of Technology Use (SOTU)
    • Assistive: Assistive Technology Device Predisposition Assessment (ATDPA)
    • Educational: Educational Technology Device Predisposition Assessment (ETPA)
    • Workplace: Workplace Technology Device Predisposition Assessment (WTPA)
    • Healthcare: Healthcare Technology Device Predisposition Assessment (HCTPA).
